Qualifications

• Experience in program management and supervisory roles is preferred.

Responsibilities

• Assesses needs to determine eligibility for intellectual disability services and creates individual service plans.
• Links individuals to services and supports per the ISP.
• Assists clients in locating and obtaining needed services, including crisis services.
• Arranges transportation for clients to access services.
• Coordinates services with other agencies involved with the client and family.
• Provides crisis intervention and supportive services to families and significant others.
• Participates on treatment teams and advocates for appropriate interventions.
• Develops community integration opportunities for living skills, vocational, civic and recreational activities.
