Job summary

Mount Rogers Community Services Board is seeking an Adult and youth support coordination/case management (SC/CM) professional to guide individuals with developmental or intellectual disabilities in accessing needed supports to live in the community and pursue their desired lifestyle.

The role emphasizes person-centered thinking, ongoing assessment, planning, and liaison with families, service providers, and state agencies. A four-year human services bachelor’s degree is required.

Responsibilities

• Coordinate with families and substitute decision makers and service providers.
