Job summary

Care Lync is seeking a dedicated Support Coordinator to develop individualized care plans and coordinate services for adults with IDD, ASD, and physical disabilities across Pennsylvania.

You will broker services, monitor outcomes, and ensure cost-effective care while assisting in locating housing, medical and social supports. Strong organizational and communication skills are required, with travel up to 50% as needed.

Qualifications

• Bachelor-level degree in a related field.
• Minimum of 3 years of experience providing case management for individuals with intellectual disabilities, autism spectrum disorders, or physical disabilities.
• Experience with Microsoft Office and state-maintained databases.
• Maintain CPR/First Aid certification and updated state related clearances.

Responsibilities

• Complete assessments to identify each participant's needs and desires for service delivery.
• Develop a Person-Centered Service Plan for each participant and modify annually or with significant changes.
• Ensure participant choice of providers by providing information for informed decisions.
• Ensure Behavioral Support Plan and Crisis Plan align with the ISP.
• Coordinate with other care coordinators to ensure seamless service delivery.
• Research and develop alternative solutions to participant needs.
• Maintain confidentiality of sensitive information.
• Attend required trainings and recertifications.
• Communicate with supervisors about participants' needs and limitations.
• Travel up to 50% as needed.
