Job summary

Care Lync is seeking a dedicated Support Coordinator / Service Coordinator / Case Manager to develop individualized care plans, coordinate services, and monitor outcomes for adults with IDD, autism, and related disabilities in Pennsylvania.

The role emphasizes comprehensive care coordination, collaboration with providers, and adherence to privacy standards, with substantial travel to serve participants in their homes and communities.

Qualifications

BS/BA degree in related course work.
Minimum of 3 years case management for individuals with IDD, ASD, or Physical Disabilities.
Experience with Microsoft Office, state-maintained databases.
Maintain CPR/First Aid certification and updated state related Clearances.
Valid Driver's License including personal vehicle insurance coverage.

Responsibilities

Complete assessments to identify participant needs and desires for service delivery.
Develops a Person-Centered Service Plan and modify annually or with significant changes.
Ensure participant choice of providers with information to make informed decisions.
Coordinate with other care coordinators to ensure seamless service delivery.
Maintain confidentiality and document communications per program requirements.
Travel up to 50% to participant locations as needed.
