Job summary

Care Lync is seeking a skilled Support Coordinator to develop individualized care plans and coordinate services for adults with disabilities in Pennsylvania. You will broker and monitor services, ensuring cost-effective, high-quality care to help clients live independently.

The role requires at least 3 years in case management, strong English communication, competency in Microsoft Office, and willingness to travel up to 50% to participant sites.

Qualifications

• Requires BS/BA degree in related field.
• Minimum 3 years experience providing case management for individuals with intellectual/developmental disabilities or autism.
• Proficiency in Microsoft Office and state-maintained databases.

Responsibilities

• Complete assessments to identify participant needs and desires for service delivery.
• Develop and modify a Person-Centered Service Plan annually or upon changes.
• Ensure participant choice of providers with information for informed decisions.
• Coordinate with other care coordinators to ensure seamless service delivery.
• Maintain confidentiality of participants' sensitive information.
• Travel up to 50% to participant locations as needed.
