Job summary

Care Lync is seeking a Service Coordinator/Support Coordinator to develop and implement individualized care plans for adults with IDD and Autism in Pennsylvania. You will coordinate services, monitor outcomes, and help clients access housing, medical, social, and community supports.

The role requires travel to participants and collaboration with other coordinators to ensure comprehensive, cost-effective care while maintaining confidentiality and compliance with regulations.

Qualifications

• Bachelor’s degree or higher in a related field.
• Experience with Microsoft Office and state-maintained databases.
• Ability to perform basic arithmetic and document accurately.
• Maintain required clearances and complete trainings as required.

Responsibilities

• Conduct assessments to identify participant needs and preferences.
• Develop and modify Person-Centered Service Plans annually or with changes.
• Ensure participants select providers and understand ISP.
• Coordinate with care teams to ensure seamless service delivery.
• Maintain confidentiality and accurate documentation.
