Job summary

Telecare Corporation in Fontana, CA is seeking a Case Manager Substance Use Counselor Certified to serve as the primary support and case manager for individuals with mental illness, focusing on recovery from substance use and addictions. The role emphasizes helping clients live as independently as possible within their community and coordinating with a multidisciplinary team.

Full-time daytime shifts are available (7:00 AM - 3:30 PM, Monday to Friday).

Qualifications

• High School Diploma or GED and relevant certification.
• Six months' experience in addictions setting.
• All certifications maintained during employment.
• Within two months of employment, ability to apply billing and learn codes.
• Sensitivity to multi-cultural populations and issues.
• Must be credentialed as a qualified mental health professional where applicable.
• Must be at least 18 years of age.
• CPR, First Aid, and CPI certification by date of employment and maintained throughout employment.
• Valid driver's license and personal vehicle insurance; willing to use company vehicle.

Responsibilities

• The Case Manager Substance Use Counselor Certified serves as the primary support and case manager for individuals with mental illness.
• Assist clients to live as independently as possible in their chosen community, focusing on substance abuse and addictions.
• Collaborate with a multi-disciplinary team to support recovery and move clients toward community integration.
