Job summary

Telecare Corporation is seeking a dedicated case manager to help individuals with mental illness live independently within their communities. The role involves direct client support in homes, hospitals, and various community settings.

Under supervision, you will coordinate services, transportation, and link clients to needed resources while maintaining compliance with program standards and safety practices.

Qualifications

• High School Graduation or G.E.D. equivalent with 3 years direct services in mental health or community services; OR Associate’s degree with 2 years; OR Bachelor’s degree in Social Services.
• At least 1 year of experience as a Substance Use Specialist.
• Valid driver’s license and personal vehicle insurance in your name.
• Willingness to use personal and company vehicle to transport clients to appointments and meetings.
• SUDP/SUDPT certification is required.

Responsibilities

• Serve as primary support and case manager to help individuals with mental illness live independently in the community.
• Provide services in clients’ homes, hospitals, and other community settings.
