Job summary

Telecare Corporation is seeking a Full Time Case Manager Substance Use Counselor at the Merrill Center CSU in Fontana, CA. You will serve as the primary support and case manager for individuals with mental illness, focusing on substance use treatment and helping clients live more independently in the community.

Shifts are Monday to Friday, 7:00 AM to 3:30 PM. Starting wage range is $25.35 to $31.32 per hour, with geographic differentials.

Qualifications

• Six (6) months’ experience in addictions setting.
• Must be credentialed as a qualified mental health professional, where applicable.
• Must be CPR, First Aid, and CPI certified (hands-on course) on date of employment or prior to providing direct client care.

Responsibilities

• Primary support and case management for individuals with mental illness, focusing on substance abuse.
• Help clients live as independently as possible in their chosen community.
• Coordinate services and assist with transportation as needed.
