Job summary

Telecare's Merrill Center CSU in Fontana, CA is seeking a Case Manager Substance Use Counselor Certified to support individuals with mental illness and substance use issues. You will help clients live as independently as possible in their chosen community, coordinating care and services with a multidisciplinary team.

The role requires CADC/CATC/SUDCC/CDP or equivalent credentials, a minimum of six months in addictions settings, and current CPR/First Aid/CPI certifications.

Qualifications

• Must have a High School Diploma or GED and CADC/CATC/SUDCC/CDP or LISAC/LASAC/LSAT where applicable.
• Six months’ experience in an addictions setting.
• Certifications must be maintained for duration of employment.
• Ability to learn billing codes within two months.
• Sensitivity to multi-cultural populations and issues.
• Must be credentialed as a qualified mental health professional where applicable.
• Must be at least 18 years old.
• Must be CPR, First Aid, and CPI certified and maintain current certification.

Responsibilities

• Serve as primary support and case manager for individuals with mental illness, focusing on substance abuse and independence.
• Provide services to clients at the Merrill Center CSU in Fontana, CA.
• Coordinate treatment planning and referrals with a multidisciplinary team.
