Job summary

Life Care Centers of America is seeking a Speech Language Pathologist (PRN) to provide direct speech therapy to patients referred by physicians. The SLP should hold MS or MA in Speech-Language Pathology, be ASHA CCCs eligible or CFY eligible, and maintain an active state license along with CPR certification.

The role involves community health involvement, documentation, and collaboration with care teams in a post-acute setting. New grads welcome; CPR must be current.

Qualifications

• Requires MS/MA in Speech-Language Pathology (SLP).
• ASHA CCCs or CFY eligibility and licensure in good standing.
• Proficiency in Microsoft Word, Excel, and e-mail is expected.

Responsibilities

• Establish, assess, and modify functional goals for patients.
• Oversee and evaluate CFYs if applicable (SLP CCCs).
• Follow treatment plans under supervision and comply with federal/state guidelines.
• Chart promptly and accurately; maintain patient confidentiality.
• Collaborate with interdisciplinary team to coordinate care.
