Job summary

Enhabit Home Health & Hospice is seeking a Speech-Language Pathologist to evaluate and provide skilled therapy in patients’ homes, under physician supervision. Settings may include single or multi-family homes, assisted living, independent living, or memory care.

The role focuses on treating language impairments, motor speech disorders, dysphagia, cognitive communication difficulties, voice disorders, and augmentative communication needs, with patients who may have had a stroke or progressive

Qualifications

• Valid state driver’s license.
• Automobile liability insurance.
• Seat belts must be worn at all times while driving.
• Dependable transportation kept in good working condition.
• Ability to drive in all types of weather conditions.
• Currently licensed as an SLP in the State of Employment.

Responsibilities

• Evaluate and provide skilled speech-language therapy to individuals in a home setting under the care of an attending physician.
• Home-based care may include single or multi-family homes, assisted living, independent living or memory care.
• Treat patients with language impairments, motor speech disorders, dysphagia, cognitive communication difficulties, voice disorders and AAC needs.
• Often work with patients who have had a stroke or progressive diseases such as Parkinson’s disease and dementia.
