Job summary

You will document thoroughly, adhere to department policies, and participate in performance improvement activities while maintaining safety and quality standards.

Qualifications

• Master's degree in Speech Pathology from an accredited program.
• Speech Language Pathologist license registered in IL or applicable state.
• Basic Life Support (BLS) for Healthcare Providers.

Responsibilities

• Interprets physician referrals and conducts speech and language evaluations.
• Develops and implements written plans of care with functional goals.
• Monitors outcomes and adjusts treatment plans accordingly.
• Plans for discharge including home programs and family teaching.
• Completes required documentation following policies and standards.
• Maintains patient safety and adheres to infection control measures.
• Communicates with physicians and care teams to optimize patient care.
