Job summary

Townsend Park Health & Rehab in Cartersville, GA seeks a Social Services Director to plan and direct the social services program within a compassionate, mission-driven nonprofit setting. You’ll coordinate family activities, provide counseling, and ensure timely MDS completion, admissions, and discharge coordination.

The role emphasizes collaboration with health and community agencies, servant leadership, and clear communication with patients, families, and staff, including grievance resolution

Qualifications

• Bachelor degree preferred in social work or related field.
• Two years supervisory experience in a hospital, skilled nursing center, or other medical center preferred.
• Proficiency with Microsoft Office Suite (Outlook, Excel, Word) desirable.

Responsibilities

• Plan, develop, organize and direct the Social Services Program.
• Coordinate and participate in family and patient activities to promote social interaction and intellectual stimulation.
• Counsel patients and families about emotional, mental, or environmental needs.
• Complete the social service portion of the MDS within 14 days of admission.
• Coordinate admissions and discharges and liaise with social, health, and community agencies.
• Facilitate Patient and Family Councils and assist with grievance resolution.
