Job summary

Greene Point Health & Rehab in Union Point, GA is seeking a Social Services Coordinator to lead the social services program and support residents and families. The role focuses on counseling, activity coordination, and interdisciplinary liaison to enhance care and well-being.

The position requires a Bachelor degree or supervisory experience in a medical setting, with strong MS Office skills and excellent communication. It is a full-time on-site role with comprehensive benefits.

Qualifications

• Minimum qualifications include a Bachelor Degree preferred or two years’ supervisory experience in a hospital, skilled nursing center or other medical center.
• Excellent organizational and prioritizing skills; proficient computer use with MS Office.
• Ability to read/write/communicate effectively with patients, families, and staff.

Responsibilities

• Plan, develop, organize and direct the Social Services Program.
• Coordinate and participate in family and patient activities to promote social interaction and stimulation.
• Counsel patients and families on emotions related to death, illness, and limitations.
• Complete social service portion of the MDS within 14 days of admission.
• Coordinate admissions and discharges; act as liaison with social, health and community agencies.
• Facilitate Patient and Family Councils; assist with grievance resolution when requested.
