Job summary

Lillian Carter Health Center in Georgia is seeking a dedicated Social Services Director to oversee the facility's social services program and ensure compassionate, resident-centered care.

You will lead planning, evaluation, and staff development, ensure compliance with federal and state regulations, and collaborate with families and interdisciplinary teams to support residents' social and emotional well-being.

Qualifications

• Bachelor’s degree in Social Work or related human services field required.
• Minimum of 1 year supervised social work experience in a healthcare setting.

Responsibilities

• Oversee planning, organization, implementation and evaluation of the Social Services Department.
• Ensure social services are delivered in compliance with federal, state and local guidelines.
• Develop and monitor individualized care plans addressing residents’ social and emotional needs.
• Provide ongoing support to residents and families during admissions, transitions and care conferences.
• Collaborate with interdisciplinary care teams to promote holistic, person-centered care.
