Job summary

Bolingreenhealth seeks a Social Services Director to plan, develop, organize and direct the Social Services Program in Georgia. You will coordinate family and patient activities to promote social interaction and intellectual stimulation.

The role requires strong communication skills and a commitment to compassionate care. A Bachelor degree in social work or two years of supervisory hospital experience is preferred.

Qualifications

• Bachelor degree in social work preferred.
• Two years supervisory experience in a hospital or medical center.
• Experience with care coordination and patient/family counseling.

Responsibilities

• Plan and direct the Social Services Program.
• Coordinate family and patient activities to promote social interaction.
• Counsel patients and families including end-of-life discussions.
• Complete the social service portion of the MDS within 14 days of admission.
• Coordinate admissions and discharges.
• Act as liaison with social, health, and community agencies.
• Facilitate Patient and Family Councils.
• Assist with grievance resolution.
• Understand documentation for Advance Directives.
• Promote the image and reputation of the System via servant leadership.
• Contribute to committees and project management.
• Perform other duties as necessary.
