Job summary

Townsend Park Health & Rehab is seeking a Social Services leader to plan, direct, and coordinate the Social Services Program. You will support patient and family activities, counsel on sensitive issues, and liaise with healthcare and community partners.

The role requires a bachelor’s in social work or equivalent supervisory experience in a hospital or skilled-nursing setting, and strong communication skills. We offer a supportive, mission-driven environment with competitive benefits.

Qualifications

• Bachelor degree in social work or related supervisory experience in healthcare preferred.
• Prior supervisory experience in a hospital, skilled nursing center, or medical center is preferred.

Responsibilities

• Plan, develop, organize and direct the Social Services Program.
• Coordinate family and patient activities to promote social interaction and stimulation.
• Counsel patients and families on feelings about death or limitations.
• Complete the social service portion of the MDS within 14 days of admission.
• Coordinate admissions and discharges.
• Act as liaison with social, health, and community agencies.
• Facilitate Patient and Family Councils as appropriate.
• Assist with grievance resolution when requested.
• Understand documentation for Advance Directives.
• Promote the image and reputation of the System by leadership and communication.
• Contribute to committees and project work within the System.
