Job summary

Outfield Healthcare Partners is seeking an Activity Director to lead and implement a comprehensive activity program for residents in a long-term care setting. You will plan, organize, and deliver social, spiritual, intellectual, and physical activities aligned with the resident care plan.

The role requires certification and licensure as an Activity Director, with at least one year of LTC experience and strong interpersonal skills to engage residents and families effectively.

Qualifications

• Certification requirements specify Activity Director credentials.
• Bachelor’s degree in recreation therapy and licensure/registration required.
• Minimum one year of activity director experience in long-term care.

Responsibilities

• Develops and delivers activity programs to promote resident engagement.
• Completes MDS sections and attends Resident Care Plan meetings.
• Conducts admissions interviews and documents activity sections of assessments.
• Plans monthly activity calendars and posts them in the center.
• Identifies resident activity preferences and participation barriers.
• Creates individualized care plans based on needs and preferences.
