TASC, serving Belleville, IL, seeks a full-time Case Manager to provide crisis intervention, care management and behavior support to foster parents and youth in care. This role brings together services, agencies, resources and other people to help clients achieve established goals, including work with high-risk caseloads.

Ideal candidates hold a bachelor’s degree in human services or related fields; CADC cert may be required.

Qualifikationen

• Crisis intervention and care coordination experience within youth and family services.
• Ability to screen and assess clients for agency services.
• Knowledge of ethics and confidentiality requirements.
• Must have valid driver’s license and reliable transportation.

Aufgaben

• Provide direct services and coordinate other services to help clients meet their established service plan goals.
• Screen and assess clients for eligibility for agency services.
• Adhere to the service hour protocol established by the agency.
• Counsel and aid family members to support the client or patient.
• Represent clients and the agency at staffing, court hearings, and provider networks.
• Provide referrals for identified needs and crisis intervention when needed.
• Document assessments, service plans, progress notes, discharge summaries and other client-related activities.
