Qualifications

• Bachelor’s degree in a related field required.
• Background check via fingerprinting and RAP Back enrollment required.
• Ability to screen for program eligibility and coordinate recovery services.
• Strong communication and documentation skills.
• Ability to work with diverse populations in a fast-paced setting.

Responsibilities

• Screen clients for eligibility and orient them to the program.
• Assist clients in establishing their recovery service plan.
• Maintain current client contact information and bi-monthly contact.
• Advise and advocate for clients with providers and community resources.
• Link clients with community resources and manage voucher usage.
• Monitor service effectiveness and authorize additional services as needed.
• Communicate progress with treatment providers, officers, and case managers.
• Develop partnerships with community agencies to increase referrals.
• Attend and facilitate meetings with community partners.
• Ensure compliance with grant requirements and train providers on voucher systems.
• Review billing records and maintain client files and case notes.
