Job summary

Harbor Light Center in Denver, CO is seeking a Temporary Recovery Case Manager to provide individualized case management for residents with substance abuse and related challenges. Under supervision, you will conduct assessments, plan services, and connect clients to housing and treatment resources.

The role emphasizes crisis intervention, documentation, and collaboration with the program team. It is part-time, non-exempt, with a 9am-2pm M/W/F schedule and ends May 2027.

Qualifications

• Enrolled in or recently admitted to a master's degree program in Social Work, Counseling, Psychology, Public Administration, Human Services, Criminal Justice, or related field.
• Strong interest in working with individuals affected by substance use, homelessness, incarceration, or other barriers to stability.
• Strong communication, organization, interpersonal and documentation skills.
• Ability to maintain confidentiality and professionalism when working with participants and staff.
• Prior experience in behavioral health, social services, criminal justice, or community-based settings preferred.

Responsibilities

• Provide direct program services to Harbor Light residents using CBT and strength-based approaches, including individual case management and group processing.
• Conduct comprehensive screenings and assessments to collect functional, environmental, psychosocial, financial, employment, housing, educational and health information.
• Assess barriers and develop individualized case plans; coordinate planning to meet short- and long-term needs.
• Provide service linkages and support systems; coordinate case conferences and monitor progress.
• Maintain documentation, discharge summaries and ensure WellSky and other assessments are up to date; advocate for residents as needed.
