Abound Health is seeking a qualified professional to join our team, supporting individuals receiving Innovations Waiver or State Funded Services. You’ll advocate for recipients and ensure high-quality care and support.

You’ll develop individualized goals, supervise staff, participate in ISP meetings, monitor billing, and maintain thorough documentation while embodying compassion and professionalism at every step.

Qualifications

• Bachelor’s degree in a related human service field with 2 years of full-time IDD experience, OR
• Bachelor’s degree in an unrelated field with 4 years of full-time IDD experience, OR
• Master’s degree in a related human service field with 1 year of full-time IDD experience.

Responsabilités

• Develop individualized short-term goals and interventions.
• Lead clinical and employee supervisory visits, ensuring the highest standards of care and compliance.
• Provide ongoing development plans and annual performance evaluations for direct care staff.
• Participate and advocate for recipients in their ISP meetings, ensuring their voices are heard.
• Monitor billing, maintain accurate documentation, and resolve grievances swiftly and compassionately.
• Represent Abound Health at community events, spreading awareness and making connections.

Qualifications:

• Bachelor’s degree in a related human service field with 2 years of full-time IDD experience, OR
• Bachelor’s degree in an unrelated field with 4 years of full-time IDD experience, OR
• Master’s degree in a related human service field with 1 year of full-time IDD experience.

Base pay is determined by factors such as work location, experience, job-related skills, and relevant education or training. In addition to base pay, total compensation may include various other forms of pay. We offer a comprehensive benefits package, including competitive health, dental, and wellness benefits, which begin on your 60th day of employment. Additionally, our 401(k) plan becomes available after 12 months of employment.
