Job summary

The Franklin County Department of Human Resources is seeking a Case Manager to determine client eligibility for TANF, Medicaid, food assistance, and other public programs. You will interview clients in person, over the phone, or in the field to assess needs and document information.

You will develop cooperative plans, explain rights and responsibilities, make referrals, monitor participation, and maintain case files while complying with local, state, and federal guidelines.

Qualifications

• Associate's degree in social work or human services, or a Bachelor's degree, or 4-year combination of related training and experience.
• Must meet background check requirements.
• Two years of experience in social work, case management or public assistance programs.

Responsibilities

• Interview clients to assess needs and eligibility for TANF and other programs.
• Gather and document financial and employment information and verifications.
• Authorize or deny participation in public assistance programs and payments.
• Develop cooperative plans to meet client needs and provide information.
• Maintain case files and documentation; attend conferences and training.
