Job summary

Evolve Therapy Services is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Blue Hill, NE. This part-time or PRN position supports a Physical Therapist in delivering patient-centered rehab services.

Responsibilities include screenings, treatment planning, and providing group and individual therapies. Requires an Associate's degree in PT and an active PTA license, with ongoing continuing education to maintain licensure.

Qualifications

• Requires an Associate's degree in physical therapy.
• Active PTA license in state(s) of practice.
• Commitment to ongoing continuing education and licensure maintenance.

Responsibilities

• Provide physical therapy services to residents under the supervision of the Physical Therapist.
• Conduct screenings to determine need for intervention.
• Assist PT in evaluations and treatment planning with short- and long-term goals.
• Conduct individual and group treatments covering strength, mobility, balance, gait, ROM and modalities.
• Assist in selecting techniques and adapting activities per resident performance.
• Monitor responses and report modifications to the Physical Therapist.
• Help establish resident schedules with the PT and team.
• Adhere to schedules and modify treatments as needed.
• Educate residents, families, and staff on therapies and equipment use.
• Document compliant and medically necessary CMS-based notes in NetHealth EMR.
