Job summary

Evolve Therapy Services seeks a part-time Physical Therapy Assistant for an in-house Skilled Nursing Facility in Grand Island, NE. You will work under the supervision of a Physical Therapist to deliver planned PT services to residents.

Responsibilities include screenings, treatment delivery, scheduling coordination, and staff training. A PTA license is required, with ongoing education to maintain licensure. This position offers flexible hours and CMS-compliant documentation supports.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in the evaluation under the Physical Therapist.
• Assist in establishing and modifying individual resident schedules with the Physical Therapist and team.
• Train residents, family and appropriate staff in the use of equipment and treatment techniques.
• Complete compliant and medically necessary documentation in NetHealth EMR as required by CMS.
