Job summary

Merrick Inc. is seeking a Program Coordinator to support adults with intellectual and developmental disabilities by coordinating services, employment goals, and required documentation.

You will manage client groups and collaborate with families, guardians, case managers, residential providers, employers, and Merrick’s program teams. The role includes visiting clients at community job sites to assess fit, attend meetings with support teams, and provide backup support within Employment Services.

Qualifications

• QDDP or Designated Coordinator as defined in federal/state statute.
• One of: (a) Baccalaureate in human services with 1 year direct care experience, (b) Associate in human services with 2 years direct care, (c) Diploma in human services with 3 years direct care, (d) Four years direct care under supervision.

Responsibilities

• Develop, write, monitor, and evaluate individualized client goals and outcomes.
• Complete and maintain client files, assessments, documentation, reports, and other paperwork.
• Meet regularly with clients and their support teams, including families, guardians, case managers, residential providers, and other service providers.
• Visit clients at community job sites to check employment status and fit.
• Maintain professional communication with clients, their support teams, employers, and internal program staff.
• Submit waiver, funding, and rate documentation and follow up to ensure accurate Service Agreements.
• Coordinate client transportation needs and other services as needed.
