Job summary

Merrick Inc. is seeking a Program Coordinator to support adults with intellectual and developmental disabilities in Minnesota.

You will coordinate services, employment goals, and required documentation, working closely with clients, families, guardians, case managers, and employers. Regular client visits to job sites, meetings with support teams, and backup support within Employment Services are key aspects of the role.

Qualifications

• Must meet one of the listed education/experience combinations for DD/Disabilities services.
• Bachelor's degree in human services with related direct care experience.
• Associate degree in human services with two years of direct care experience.
• Diploma in human services with three years of direct care experience.
• Four years of direct care experience under supervision of a qualified professional.

Responsibilities

• Develop, write, monitor, and evaluate individualized client goals and outcomes.
• Complete and maintain client files, assessments, documentation, reports, and other required paperwork.
• Meet regularly with clients and their support teams, including families, guardians, case managers, residential providers, and other service providers.
• Visit clients at community job sites to check on progress and fit, identify concerns, and adjust support.
• Maintain professional communication with clients, support teams, employers, and program staff.
• Submit waiver, funding, and rate documentation and follow up to ensure accurate Service Agreements.
• Coordinate client transportation and other services as needed.
