Avera Sacred Heart Pavilion is seeking a Speech-Language Pathologist to evaluate patients, develop treatment plans, and provide therapy to address speech, language, cognition, voice, and dysphagia across home-based settings as needed.

The role emphasizes multidisciplinary collaboration with a focus on quality care, documentation, and patient-family education in a fast-paced clinical environment.

Qualifications

• CCC-SLP certification or enrollment in CFY with certification within one year of employment.
• Active license as a Speech-Language Pathologist in the state of practice.

Responsabilités

• Evaluates patients with formal and informal assessments to determine intervention needs.
• Develops and implements individualized treatment programs for patients.
• Plans therapy considering patient’s physical, emotional and intellectual capabilities in coordination with the care team.
• Provides direct therapy services across various settings and strategies.
• Documents evaluation results, treatment plans and progress notes.
• Communicates with patient, family, physician and team about status and needs.
• Supports program development and supervises students affiliating with universities/colleges.

CCC-SLP certification / CFY under supervision
