Job summary

CommonSpirit Health at Home is seeking a Speech Language Pathologist to join the VNA Home Health team. The SLP will assess, diagnose, and treat patients with speech, language, communication and swallowing disorders in patients' homes, with individualized treatment plans and collaboration with families and other health professionals.

Qualifications

• Master's degree in Speech-Language Pathology.
• Active state SLP license and CCC-SLP (ASHA).
• CPR/BLS certification required.
• Minimum 1 year clinical SLP experience.
• Valid driver's license.
• Must comply with applicable laws and regulations.

Responsibilities

• Provide therapy to patients in their homes to improve speech, language, cognitive-communication, and swallowing.
• Customize therapy strategies to meet individual patient goals.
• Document patient progress and treatment outcomes to regulatory standards.
• Train family members to support home-based therapy.
