Job summary

ChenMed, a physician-led primary care provider, seeks a Primary Care Physician (PCP) to deliver age-friendly care in our Chicago centers. You will manage a patient panel, lead a care team, and focus on outcomes, cost-effectiveness, and patient relationships.

You will collaborate with hospitalists and APPs, participate in risk and quality programs, and maintain high standards of clinical care and patient satisfaction. English fluency required.

Qualifications

• MD or DO in Internal Medicine, Family Medicine, or Geriatrics; board certified preferred.
• Active state license and DEA license required or obtainable.
• BLS certification within 90 days of employment.

Responsibilities

• Provide care for older adults with acute and chronic illnesses.
• Take accountability for patient outcomes and coordinate with specialists as needed.
• Collaborate with hospitalists, case managers, and care teams for transitions.
• Lead a care team and engage in active recruitment and center leadership.
• Ensure compliance with guidelines, quality measures, and policies.
