Oak Street Health in Aurora, CO is seeking a Nurse Practitioner, Advanced Practice Provider to deliver value-based primary care to adults on Medicare. NP will practice in outpatient clinics, collaborate with physicians, and manage longitudinal care with a multidisciplinary team.

The role requires MSN/DNP or NP certificate, board certification, active NP license and DEA, and US work authorization. The position offers a comprehensive benefits package and a 40-hour work week.

Qualifications

• Requires MSN, DNP or NP certificate with board certification.
• Active, non-probationary NP license in good standing.
• Active DEA license.
• US work authorization.
• Experience in value-based primary care is preferred.

Responsabilités

• Provide exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer annual wellness visits and health risk assessments.
• Coordinate care with providers, facilities, and testing sites.
• Lead population health initiatives with the care team.
• Assist with phone triage and member outreach.
• Educate patients on conditions, plans, and treatments.
• Participate in Oak Street Health promotions.
• Conduct home visits as needed.
