Job summary

Oak Street Health Fayetteville, NC seeks a Nurse Practitioner to deliver value-based primary care to adults on Medicare. The NP will see patients independently within a collaborative care team and coordinate with physicians to build lasting relationships.

This full-time role reports to the Center Medical Director, includes office visits, wellness visits, care coordination, and participation in population health initiatives. A MS-NP, national certification, and active DEA license are required.

Qualifications

• Master's, Doctorate, or NP certificate required.
• Active state NP license and DEA license required.
• US work authorization required.

Responsibilities

• Provide effective and equitable value-based primary care to adults on Medicare.
• See patients independently and collaborate with physicians.
• Conduct annual wellness visits and health risk assessments with thorough documentation.
• Care coordination with other providers and facilities.
• Participate in population health initiatives and team activities.
• Educate patients about conditions, plans, and treatments.
• Possibly conduct home visits as needed.
