Job summary

Housing Works is seeking a Prevention Navigator to engage participants across two harm reduction program sites in New York City. You will perform outreach, screening, and care coordination, helping people access prevention services, treatment, healthcare, housing, benefits, and other resources.

Travel between locations is required and confidentiality is essential. The role emphasizes low-threshold, trauma-informed engagement and collaboration with a multidisciplinary team to promote continuity

Qualifications

• Bachelor’s degree in a health field is preferred.
• Minimum 1 year of relevant nonprofit experience.
• Knowledge of HIV/AIDS, homelessness, substance use and LGBT youth issues.

Responsibilities

• Provide direct participant engagement, navigation, and support across two harm reduction sites.
• Conduct screenings and assessments for prevention services and social determinants of health.
• Educate participants on harm reduction, HIV/STI prevention, and wellness topics.
• Facilitate referrals to primary care, substance use treatment, housing, benefits, and HIV care.
• Support enrollment and retention in prevention and treatment programs.
• Document encounters in electronic health records and data systems.
