Job summary

Talented Medical Solutions seeks a compassionate Physical Therapist (PT) to provide high-quality rehabilitation services in a healthcare setting. You will evaluate patients, develop individualized plans, implement interventions, and monitor progress to restore mobility and reduce pain.

The role requires an active PT license, a DPT or equivalent, and experience in acute care, outpatient, inpatient rehab, SNF, or home health.

Qualifications

• Active PT license in the state of practice.
• Current BLS/CPR certification as required by policy.
• DPT degree or equivalent per state regulations.
• Experience in acute care, outpatient, inpatient rehab, SNF or home health.
• Strong clinical assessment, communication, documentation and patient education.
• Ability to work independently and with an interdisciplinary team.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide therapeutic interventions to improve mobility and function.
• Monitor progress and modify plans; document evaluations and outcomes.
• Educate patients and families on treatment plans and home exercises.
• Collaborate with physicians and teams to coordinate care.
• Maintain accurate clinical documentation and regulatory compliance.
• Treat patients across assigned settings and age groups.
• Perform other job-related duties as assigned.
