Job summary

PATH (People Assisting The Homeless) is seeking an Outreach Case Manager in Los Angeles, CA to support clients experiencing homelessness by providing intake, ISP development, and linkage to housing, health services, and benefits. You will work within the Downtown Center BID and Harbor UCLA offices with a focus on coordinated care.

Qualifications

• Experience conducting intake and developing service plans.
• Knowledge of HIPAA and client confidentiality.
• Ability to assess housing goals and access services.
• Strong communication and collaboration with partners.

Responsibilities

• Conducts intake and develops an ISP addressing housing goals and income; covers health, mental health, substance use, financial resources and social support.
• Provides referrals and linkages to resources to help participants achieve ISP goals.
• Completes program-specific assessments (e.g. VI-SPDAT).
• Evaluates progress with a 90-day review and maintains participant files per PATH policies.
• Maintains confidentiality of participant files per HIPAA and guidelines.
• Develops therapeutic relationships to engage clients with housing goals and supports.
• Provides crisis intervention and risk assessment to increase safety and independence.
• Uses motivational interviewing and trauma-informed care in service delivery.
• Keeps up-to-date knowledge of community resources to offer wrap-around services.
