Job summary

PATH is seeking candidates for the Case Manager - Outreach role at the Los Angeles, CA 90805 area to help clients move out of homelessness and toward stable housing. You will conduct intake, develop individualized service plans, provide referrals, and coordinate with the outreach team.

The position requires full-time, non-exempt work with driving responsibilities, and a Monday–Friday schedule. Expect a salary hourly between $23.24 and $28.28 with 24 pay periods per year and a comprehensive

Responsibilities

• Conduct intake and develop ISP addressing housing and income goals.
• Provide referrals and support to resources to meet ISP objectives.
• Complete program assessments (e.g. VI-SPDAT) and 90-day reviews.
• Maintain documentation and ensure confidentiality per HIPAA and PATH policies.
• Ensure complete and accurate participant files with required documents.
