Job summary

Special Service for Groups, Inc. in Los Angeles is seeking an Outreach Case Manager for the HOPICS division. The role provides professional case management to homeless consumers, helping them obtain housing, benefits and health services in the field.

Under supervision of the Program Supervisor, the incumbent will document in HMIS, participate in trainings, and coordinate field-based services with a multidisciplinary team to improve client outcomes.

Qualifications

• Some college credits or at least two years working with homeless individuals.
• Ability to work with diverse cultural, ethnic, and socio-economic backgrounds.
• Experience working in the field; effective written and oral communication; MS Word/Excel/Internet.
• Experience with co-occurring mental health, disabilities or substance use issues.
• Ability to report in accordance with Tarasoff, child abuse, vulnerable adult abuse and safety requirements.
• Professional experience in a multidisciplinary team.
• If in recovery, able to coordinate documentation submissions.
• Minimum of three years of being drug- and alcohol-free.
• Ability to verify employment eligibility and pass background clearance.
• TB test within 3 months prior; CPR and First Aid within 30 days of employment.

Responsibilities

• Conduct immediate needs assessments and develop comprehensive treatment plans with MDT input.
• Provide field-based case management and linkages to housing, benefits, health and social services.
• Document daily per standards; maintain HIPAA and CFR records; use HMIS.
• Coordinate transportation and field operations; maintain client liaison with community partners.
• Attend mandatory meetings, trainings, and supervision; ensure program contractual compliance.
• Represent the agency professionally at meetings and community events; maintain client records.
