Job summary

Surprise Health & Rehabilitation is seeking a dedicated Full Time Outpatient Occupational Therapist to join our team. You will provide therapy services in skilled nursing facilities, independent living and home settings, with a flexible schedule that supports work-life balance.

Qualifications

• Licensed Occupational Therapist as required.
• Open to applicants of all experience levels.

Responsibilities

• Screen/evaluate patients with cognitive/ADL impairments and develop care plans.
• Use standardized assessments and evidence-based practices in interventions.
• Provide rehabilitative, skilled treatment per regulatory requirements.
• Administer therapies including ADL retraining, exercises, modalities.
• Monitor progress and adjust treatments in the clinical record.
• Evaluate outcomes and adjust services; refer as needed.
• Provide counseling to patients, families, and caregivers.
• Communicate progress with supervisor and health team.
• Provide comprehensive discharge summaries as required.
