Surprise Health & Rehabilitation is seeking a Full Time Outpatient Occupational Therapist to join our team in Surprise, AZ. You will treat patients across settings, including skilled nursing facilities, independent living, and assisted living, and may work in inpatient, outpatient, or home-based contexts.

Responsibilities

• Screen/evaluate patients with cognitive and/or ADL disorders and develop a care plan per standards.
• Use standardized assessments and evidence-based practice to support interventions.
• Provide rehabilitative, skilled and medically necessary treatment per requirements.
• Administer procedures such as ADL retraining, therapeutic activities, exercises, modalities and assistive devices.
• Assess progress and adjust treatments in the clinical record per requirements.
• Evaluate outcomes and modify services; refer as needed based on results.
• Provide consultation to patients, families, caregivers and other providers.
• Communicate with supervisor and health team about progress and plans.
• Provide comprehensive discharge summaries per regulatory requirements.

OUTPATIENT OCCUPATIONAL THERAPIST - FULL TIME - SURPRISE HEALTH & REHABILITATION - SURPRISE, AZ

We are seeking a dedicated and compassionate Full Time Outpatient Occupational Therapist to join our team at Surprise Health & Rehabilitation. This position offers an excellent opportunity to make a meaningful impact on patients’ lives by providing therapy services in a variety of settings, including skilled nursing facilities, independent living, and assisted living facilities. You will treat patients in both inpatient and outpatient settings, often within the comfort of their homes.

Full Time: Flexible schedule with work-life balance.

Wage: $40 - $50/hour based on experience.

Key Responsibilities

• Effectively screens/evaluates patients with cognitive and/or ADL physical functioning disorders and develops appropriate plan of care following all regulatory and clinical practice standards.
• Utilizes standardized assessments and evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements.
• Administers various procedures as part of the rehabilitation plan including but not limited to: ADL retraining; therapeutic activities; therapeutic exercises; modalities; use of supportive and assistive devices; etc.
• Assesses patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
• Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.
• Effectively communicates with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
• Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.
