Qualifications

• Degree in Occupational Therapy from an accredited program.
• NBCOT certification or eligibility per state.
• Current OT license in practice state.

Responsibilities

• Evaluate and treat OT patients in SNF/long-term care.
• Develop and implement personalized care plans.
• Collaborate with care team to ensure quality outcomes.
• Maintain documentation to regulatory standards.
• Ensure physician orders are obtained and current.
