Surprise Health & Rehabilitation is seeking a full-time Outpatient Physical Therapist to join our team in Surprise, AZ. You will treat patients in outpatient settings and within home-based contexts, contributing to a comprehensive therapy program across skilled nursing, independent living, and assisted living facilities.

The role offers a flexible schedule with a competitive hourly wage ($40–$46), based on experience, and opportunities to work in diverse settings with a patient-centered approach.

Qualifications

• Physical Therapist license is required.
• Open to Physical Therapists at all experience levels. New Grads are welcome!

Responsibilities

• Conduct screening and evaluation of patients, develop care plans adhering to standards.
• Deliver rehabilitative and medically necessary treatment interventions.
• Administer procedures as part of the rehabilitation plan, including manual techniques and wound care.
• Provide consultation to patients, families, and caregivers regarding physical disorders.
• Generate discharge summaries in compliance with regulatory requirements.
