Surprise Health & Rehabilitation is seeking a dedicated Full Time Outpatient Physical Therapist to join our team in Surprise, AZ. You will treat patients in both inpatient and outpatient settings, including home-based care, with a flexible schedule and a compensation range of $40–$46 per hour based on experience.

Physical Therapist license is required.
Open to Physical Therapists at all experience levels. New Grads are welcome!

Responsibilities

Screen and evaluate patients with physical functioning disorders and/or wound care, developing care plans.
Deliver rehabilitative, skilled, and medically necessary treatment interventions.
Administer manual techniques, ambulation, therapeutic exercises, modalities, devices, and wound care.
Provide consultation and counseling to patients, families, caregivers, and other service providers.
Generate discharge summaries of services provided in compliance with regulations.
