Qualifications

Responsibilities

• Conduct assessments and develop individualized treatment plans.
• Provide therapy services in skilled nursing facilities, independent living, and assisted living facilities.
• Monitor patients’ progress and document responses to treatment.
• Collaborate with a multidisciplinary healthcare team to provide comprehensive care.
• Educate patients and families on treatment plans and home exercise programs.
