Responsibilities

• Coordinate the full continuum of care by assessing needs and developing service plans.
• Build meaningful client relationships through active listening and supportive counseling.
• Manage an assigned caseload and monitor service utilization and progress.
• Develop individualized treatment and service plans promoting wellness and independence.
• Coordinate essential services and resources including housing, transportation, and referrals.
• Monitor progress and document outcomes; determine when to close cases in consultation.
