Position Summary
Make a meaningful difference in the lives of individuals navigating behavioral health challenges. As an Outpatient Case Manager, you’ll serve as a trusted partner throughout the client’s care journey—assessing needs, coordinating services, advocating for resources, monitoring progress, and helping clients build the skills and support systems needed to achieve greater wellness and independence.
You’ll collaborate with clients, families, providers, and community partners to ensure care is safe, timely, effective, efficient, equitable, and truly client-centered.
What You’ll Do
- Coordinate the full continuum of care by assessing client needs, developing service plans, implementing interventions, monitoring progress, and evaluating outcomes.
- Build meaningful client relationships through consistent contact, active listening, supportive counseling, and a strengths-based approach.
- Manage an assigned caseload while monitoring service utilization, client progress, and changing needs.
- Develop individualized treatment and service plans that promote wellness, stability, independence, and recovery.
- Coordinate essential services and resources, which may include crisis intervention, advocacy, medication monitoring, transportation assistance, financial budgeting, housing support, and referrals to community resources.
- Help clients navigate complex systems by coordinating care, sharing information appropriately, and connecting clients with the services they need.
- Monitor and document progress while determining appropriate case closure in consultation with a supervisor and/or treatment team.
- Participate actively in treatment teams, contributing observations and recommendations regarding client progress and changes in service needs.
- Maintain accurate and timely documentation that meets program, funder, credentialing, agency, and regulatory requirements.
- Provide education and support, including assisting with group education or curriculum when needed.
- Advocate for client needs and promote autonomy, empowering individuals to take an active role in their health and well-being.
- Stay current with professional standards, agency policies, protocols, regulations, and required training.
- Perform other duties as assigned.
