Bridge Home Health & Hospice is seeking an Occupational Therapist to implement standards of care for therapy services in the home setting. You will evaluate functional status, safety in the home, and plan care with physicians, while teaching patients daily activities and supervising supportive staff.

The role requires California OT licensure, NBCOT registration, CPR certification, and at least two years' clinical OT experience, with strong communication and organizational skills.

Formación

• Graduate from an OT curriculum accredited by the relevant boards and associations.
• Registered with NBCOT as an Occupational Therapist.
• Licensed to practice occupational therapy in California by the state Board.
• Minimum two years of clinical OT experience.
• Strong verbal and written communication and organizational skills.
• Current CPR certification.
• Valid California driver's license with a reliable vehicle.

Responsabilidades

• Evaluate patients' functional status and home environment for safety and independence.
• Develop treatment plans and goals; communicate care plans to physicians.
• Teach new skills or retrain daily activities lost due to illness or injury.
• Fabricate splints and train patients in adaptive equipment use.
• Train patients in use of prosthetic/orthotic devices when indicated.
• Maintain clinical records and report changes to the physician.
• Coordinate with other care team members to ensure comprehensive plans.
• Supervise certified occupational therapy assistants per policy.
