Job summary

Bridge Home Health & Hospice in Fresno, California seeks an Occupational Therapist to implement standards of care and ensure compliance with the Service Agreement. The OT will evaluate patients, develop treatment plans, and coordinate with physicians and care teams to promote functional independence.

The role includes supervising aides, providing patient education to families, and maintaining accurate clinical records in accordance with organization policy.

Qualifications

• Graduate of an occupational therapy curriculum accredited jointly by the appropriate bodies.
• Registered with NBCOT (National Registration Examination).
• Licensed as an occupational therapist by the California Board of Occupational Therapy.
• Two years of clinical experience as an occupational therapist.
• Demonstrates good verbal and written communication, and organization skills.
• Possess and maintains current CPR Certification.
• Must possess a valid California Driver’s License with automobile.

Responsibilities

• Evaluates patient’s functional status and home environment; identifies equipment needs; contributes to the plan of care.
• Performs work capacity evaluation for patients returning to work; refers to vocational training as needed.
• Develops treatment program and communicates plan of care to physicians and care teams.
• T conducts therapy to restore or improve function and may teach/ retrain daily activities.
• Maintains clinical records and reports changes in patient condition.
• Supervises certified occupational therapy assistants per policy and state regulations.
• Instructs family/caregivers and other personnel in the treatment regime; coordinates with care team.
