Job summary

Bridge Home Health & Hospice is seeking an Occupational Therapist contracted through the Organization to implement standards of care for occupational therapy services and to ensure adherence to the Service Agreement. Direct patient care will be performed as defined by the California Occupational Therapy Practice Act.

Responsibilities include evaluating functional status, formulating the plan of care, teaching and retraining daily activities, fabricating splints, supervising assistants,

Qualifications

• Graduate of an occupational therapy curriculum accredited jointly by the AMA and AOTA.
• Licensed as an occupational therapist by the California Board of Occupational Therapy or eligible for licensure.
• Registered with NBCOT.
• Current CPR certification.
• Valid CaliforniaDriver’s License with reliable vehicle.

Responsibilities

• Evaluates patient’s functional status and home environment for barriers to independence.
• Develops treatment plan and communicates plan of care to physicians and team.
• Teaches new skills and retrains daily activities as needed.
• Supervises certified occupational therapy assistants and home health aides as required.
• Maintains clinical records and coordinates care with the care team.

Education

Graduate OT curriculum (AMA and AOTA accredited)

California OT License

NBCOT registration

CPR certification

Valid California driver's license
