Job summary

Bridge Home Health & Hospice seeks an Occupational Therapist to implement standards of care for OT services in a home health setting. You will evaluate patients, develop plans of care, and coordinate with physicians and staff to ensure quality outcomes.

Responsibilities include functional assessments, work capacity evals, teaching activities to patients and families, and supervising aides per policy. California OT license and CPR certification required; valid driver's license and reliable

Qualifications

Graduate of an occupational therapy curriculum accredited by the relevant associations.
Registered with the National Registration Examination and licensed by the California Board of Occupational Therapy.
Two years of clinical experience as an occupational therapist.

Responsibilities

Evaluates patient's functional status and home environment to develop total plan of care.
Performs work capacity evaluations and refers to vocational training as needed.
Develops treatment programs and communicates plan of care to physicians and staff.
Instructs patients and caregivers in daily activities and use of adaptive equipment.
Maintains clinical records and reports changes to the physician.
